Registration Form

Thank you for registering your daughter at Methodist Ladies’ College. We look forward to partnering with you in growing your daughter's unique strengths in a nurturing environment where she can make friends for life.

To complete this Registration Form we will need a scanned copy of a few important documents so please keep the following documents of your daughter on hand:

Birth certificate
Current Immunisation Record - AIR Statement, generated no more than 2 months ago. Please note, MLC requires an up-to-date immunisation record prior to commencement as a prerequisite.
Last two years' school reports (if attended school previously)
Years 3, 5, 7 and 9 NAPLAN results (if applicable)
If both parents were born overseas, proof of your daughter’s residency (visa, citizenship certificate or Australian passport - photo page only)
Any relevant specialist reports relating to your daughter's educational needs (if applicable)
Any court or parenting orders pertaining to the guardianship of your daughter (if applicable)

Once you have submitted this form, you will be taken to our secure credit card payment page provided by Westpac to pay your daughter's non-refundable AUD$150 Registration Fee.
